Genesis Accepts the Following Insurance Plans ONLY: 

• Blue Cross/Blue Shield PPO, Medicare, Medicare Advantage PPO, and Aetna PPO plans

We are not contracted with any HMO or Medicaid plans.

Medicaid requires all Providers to be in network for prescription coverage. Our prescribers are not authorized to issue prescriptions for Medicaid Patients.

As of January 1, 2022 all out of network plans are subject to our self pay rates below. Our Financial policy states all patients must have a form of payment on file. Our practice managment system is encrypted and HIPAA compliant.

Upon request, at end of each session, our Patient Relations Staff will provide a copy of your encounter form which includes all the pertinent information for patients to submit to out of network insurance provider for reimbursement. Plans do vary, please contact your insurance company for proper reimbursement protocol.

We do offer competitive self pay rates for 1st initial evaluation and follow-up appointments.  *As of January 1, 2023 these rates will increase by $15 per session.

Once we verify your insurance and make ensure the therapeutic relationship is a match for both Client and Clinician, Our Business office will email you a new patient packet to complete on-line. Once your registration packet is sent to you through Adobe Acrobat Pro, you completed paperwork will be return to us with your e-signature. We will call you to set up your first initial appointment. We can not schedule an appointment prior to receiving your completed New Patient Registration Packet.
